are Planning Conference is held for each new resident within three weeks after admission and routinely on a   quarterly basis or upon a condition change. The purpose of the conference is to develop a plan to meet the needs of the resident including all aspects of life; physical, emotional, social and spiritual.

Members of the conference team include representatives from the nursing, dietary, activities, and social services departments. A letter is given to the resident or mailed to a family member inviting them to attend the Care Planning Conference. It is a time to share        concerns, ask questions, and set goals. We strongly encourage both resident and family to attend the conference. Your input is very    important!

We regret that we are unable to alter times and dates and must limit each session to fifteen minutes due to the number of people we must review each day.
