- For each additional resident per unit, $700 per month is assessed
- An additional $200 per month is assessed as each level of service is increased
- Level of services is based on a point system from the Functional Assessment
- Assessment tools and Occupancy Agreement are available upon request
Levels of Service
Level I Service
Basic Service Includes:
- Two meals/day
- Cable television (SMU)
- Use of common areas
- Wellness checks
- Laundry facilities
- Storage areas
- Secure building
- Medication management
- All utilities except telephone
- Light housekeeping
- Planned social activities
- Pool
- Assistance with Activities of Daily Life (ADLs)
- Access to beauty/barber shop
- Health assessment/monitoring
- Nurse on-call 24 hours a day
- Priority admission to St. Luke Nursing Home

Level II & III Service
As Needs increase, services may increase to Level II & III, which may include any of the following:
- Daily assistance with ADLs
- Daily housekeeping
- Daily medication management
- Monitoring of medical needs
- Three meals a day
- Manageable incontinence care
- Assistance with transfers or ambulation
