Care Planning Conference is held for each new resident within three weeks after admission and routinely on a quarterly basis or upon a condition change. The purpose of the conference is to develop a plan to meet the needs of the resident including all aspects of life: physical, emotions, social, and spiritual.

Members of the conference team include representatives from the nursing, dietary, activities, and social services departments. A letter is given to the resident or mailed to a family member inviting them to attend the Care Planning Conference. It is a time to share concerns, ask questions, and set goals. We strongly encourage both resident and family to attend the conference. Your input is very important!
