St. Luke Lutheran Nursing Home employs a full-time state-licensed social worker. The social service department works to maintain and improve each resident's ability to manage everyday physcial, mental, and psychosocial needs.
Contact is maintained with families in an effort to keep the appropriate parties abreast of important information pertaining to the resident. The social service department also makes referrals to outside agencies for services and aspects with discharge planning.
